My Care Journey

Use our My Care Journey interactive system to help you explore the different options available.
This system will help you find the services and information available to help you maintain as much independence as possible
It will take you through the processes of accessing these services either independently or by requesting an assessment.
Our aim is to help you to
  • live in your own home for as long as you wish
  • be an active member of your community
  • be able to choose and have easy access to the type of help you need, when you need it
  • keep safe and secure, and enjoy a good quality of life.
The right information and advice at the right time can support you in doing this.

Health and wellbeing information

We offer a wealth of information and opportunities about different areas concerning your health and wellbeing.
This information will be useful to you whether you are fit and well and aim to stay that way, or if you have some health problems or care needs and you want to prevent things getting any more difficult.
The information is organised into the different sections:
  • My Care Journey- provides an interactive opportunity to explore what you can expect when seeking help from Adult Care. It takes you through the different steps along the way and links you to information related to the path you are exploring.
  • Getting out and involved - gives you information and pointers about a range of opportunities and things you can do in your local area and community including leisure activities, forums you can join and information about volunteering.
  • Help to live at home - presents useful information to assist you to continue to live at home with some help including information about help with personal care, bathing and adapting your home.
  • Keeping safe - offers practical help and advice on how to keep safe in and around your home including information about domestic abuse, neglect and doorstep safety.
  • Accommodation and housing- gives you information on different types of accommodation and housing related support services available including advice on choosing a care home, contacts for your local district/borough councils and Supporting People information.
  • Money and legal matters - includes information on things like benefits and tax credits, managing someone else's affairs and the Mental Capacity Act. There is also a directory to help seek help and advice on managing money and debts.
  • Caring for someone - provides information on help and support for carers, Adult Placement Schemes and Carer Emergency Cards.
  • Health and wellbeing- gives useful information about staying well, plus conditions and illnesses and links with NHS information and services.
  • How Adult Care works - provides operational and other information about social care for adults in Derbyshire.

Your guide to services

Our 'Your Guide to council, health, voluntary and community services in Derbyshire 2012' is available to view or download from this page. The Guide covers a range of services that will help you live independently and well, with contacts for your local councils, crime and safety, independent living and care, housing, disability support and many other subjects.
